We are looking for Enthusiastic people to boost our existing community Therapy services as part of winter pressures initiative. Our services cover a wide geographical area including Fareham & Gosport, Havant and Waterlooville and Petersfield and Bordon.We are looking for therapists interested in working in both community and rehabilitation inpatient settings.

The applicant is afforded structured professional supervision and an annual appraisal.

These positions are designed to contribute to avoiding hospital admission, facilitating early discharge from hospital and maintaining patients in their own homes. Clients accessing this service will be predominantly (but not exclusively) in the elderly care group (over 65). This post offers the opportunity to work closely with all members of the MDT both in primary, secondary care, Adult Services as well as working closely with care/nursing home staff.

The successful candidates would complete a period of induction and will be required to assume responsibility for coordinating a plan of care and rehabilitation, working within a scope of practice autonomously as a registered practitioner. The successful applicants will be expected to lead on care planning and identification of person centred rehabilitation goals. The applicant will need to possess excellent communication skills and be competent in linking with the larger multidisciplinary team. There will be the opportunity to develop service development skills, including supervision.

The successful applicant will be proficient, knowledgeable and competent. Educated to Degree or Diploma level in Occupational Therapy or Physiotherapy and hold HPC registration. They will continue to develop in their role through the acquisition of further skills, knowledge and competencies, to be determined by the needs of patients, the service and the team.

This job involves a proportionate amount of time driving, you will need to be a car driver and have access to a car for work.
