PCN First Contact Physiotherapist

We are looking for experienced Physiotherapists to work in our GP Practices across Synergy Primary Care Network (PCN) as MSK First Contact Physios. This exciting new role (Band 7) will be joining a growing team that includes Clinical Pharmacists and Social Prescribing Link Workers. Successful candidates will have a named GP Supervisor, access to peer learning developmental opportunities.



Successful candidates will have experience of initial assessments and creating treatment plans. One of our priority focus areas will be around frailty and prevention of falls. Experience in this area would be an advantage.

YOU SHOULD BE:

This role is new to primary care, and we envisage the role to be one to enhance the patient care offering. This role will be a face to face clinical role assessing new acute issues, as well as ongoing care and support for existing conditions in a preventative approach. The role will also involve upskilling other professionals to share learning.

Synergy Primary Care Network. Based in the heart of Hillingdon, West London. Synergy PCN consists of four member practices: Central Uxbridge Surgery, Hillingdon Health Centre, Belmont Medical Centre and Brunel Medical Centre.



Synergy PCN is one of six member PCNs of The Confederation, Hillingdon (CIC); a Community Interest Company formed from 43 GP practices in Hillingdon.

Improve patient care for the patients of our member practices, which is very closely matched to the population of the London Borough of Hillingdon.

Make General Practice in Hillingdon sustainable and an attractive place to work through collaboration, the provision of support services, development of the underpinning infrastructure and support and development of the primary care workforce releasing time for hard pressed clinicians.



Provide a vehicle through which General Practice can work at scale to provide efficient and effective patient care.
