An opportunity has arisen for a Band 5 Physiotherapist to join the therapy team working into Sheffield Intermediate Care Beds service. The service is currently based in one of four private sector care homes and provides a short term community based in-patient facility for people who are unable to return home after a hospital admission. The service aims to enable patients to return home with appropriate support as early as possible by providing a range of person centred reablement and rehabilitation as well as complex discharge planning intervention.

 

Each unit has an MDT with input from Occupational Therapy, Physiotherapy, Therapy Assistants, Nurse Case Manager, Social Care Managers, Consultant Geriatrician, GP, Nurses and Support Workers. There are good communication systems and established interdisciplinary shared learning within the service. Although the successful candidate will begin work in a designated base there will be an expectation to work flexibly across the units, providing cover where required.

 

A diploma or BSc in Physiotherapy and current HCPC registration are essential. Specific post graduate Physiotherapy experience in the rehabilitation of older people, experience of working in an intermediate care service and a good understanding of current issues impacting on intermediate care are desirable.
