An opportunity has arisen with the Long Term Neurological/Stroke team for a Band 6 Physiotherapist, temporary to cover maternity leave.

The Community Neurological Rehabilitation Service aims to provide rehabilitation for adults who have a long term neurological condition or following a Stroke and who have a Bassetlaw GP. The service aims are to promote independence, prevent hospital admission or admission to care, support early discharge from hospital and to help people achieve their rehabilitation goals.

The successful candidate will take a lead role to plan, deliver, review and evaluate advanced rehabilitation/care programmes to patients who may have neurological complex health care need. They will undertake comprehensive assessment of clients including those with complex presentations using investigative skills and to formulate individual specialised patient action plans with clear goals, using clinical reasoning based on sound theoretical knowledge and experience utilising a wide range of treatment skills and options.
