An exciting opportunity has arisen to join the Admission Avoidance and Discharge Service (AADS) multidisciplinary team in Tower Hamlets. The team prevent unnecessary admission to hospital and works with the Royal London Hospital to facilitate discharge using a "discharge to assess" model.
There will be opportunities to work across the Rapid Response and Intermediate Care teams and in future to rotate within the wider community therapy services provided within Tower Hamlets, in which nursing, therapy and support staff work in multidisciplinary teams across four localities, liaising closely with local GPs. The reporting process shifts and responsibilities would vary pending on the rotation.
